Application of 3.0T MRI in Breast Cancer

[Abstract]

Along with the rise of living standard, late procreation, usage of contraceptive drug, incidence of breast tumor seemed to obviously step up. The surgical operation combined with neoadjuvant chemotherapy in early stage could obviously raise five-year survival rate, so early diagnosis played an important role. Currently, diagnosis and general investigation of breast cancer mainly depended on imageology, including mammography, near infrared Scanning and ultrasonography. But these methods had various disadvantages, such as the poor discrimination of soft tissue. Magnetic Resonance Imaging (MRI) can display perspicuously the structure of mammary gland with standout soft tissue discrimination perspicuously and harmlessly, so it had already been used to diagnose breast diseases. Nevertheless, the application of MRI in the diagnosis of breast diseases is still in a beginning stage, lack of research on large sample. The recognition of MRI feature about breast diseases was deficient. In this research 71 cases with breast tumor underwent MRI scan, including SE sequence, diffusion-weighted imaging and dynamic Contrast-enhanced MRI since 2006. We Compared MRI appearances with mammography, near infrared Scanning, ultrasonography and pathology, Summarized MRI characteristics of breast lesions especially of breast tumors, and evaluated the value of MRI to discriminate local recurrence from postoperative scar after breast conservation therapy.ExperimentⅠApplication of the routine MRI and DWI in breast cancerObjective: To investigate the characteristic of routine MRI and DWI of the breast tumor. Methods: Scanned 55 patients suffered breast tumor with routine MRI and DWI using Magnetom trio# tim 3.0T MRI manufactured by Siemens. Analyzed the results of the MRI of the breast tumor along with the results of other examination including mammography, near infrared scanning and ultrasonography and pathology. Results: 55 patients had 37 malignant and 18 benign lesions. Breast cancer showed as irregular–shaped masses nodes without identified border and with uneven low T1 signal and uneven high T2 signal. Benign breast tumor appeared as round mass with smooth border with even low T1 signal and even high T2 signal. The ADC value of breast cancer, benign breast signal and normal breast gland is (1.02±0.17)×10-3(mm2/s), (1.78±0.24)×10-3(mm2/s),(1.97±0.26)×10-3(mm2/s) respectively. The ADC value of breast cancer is lower than that of benign tumor and normal breast gland(p<0.05), and the ADC value of benign tumor has no significant difference with that of the normal breast gland.ExperimentⅡApplication of dynamic contrast enhanced MRI in diagnosis breast cancerObjective: To investigate the value of dynamic contrast enhanced MRI in diagnosis of breast lesions. Methods: 55 patients were enrolled in this study who were confirmed as breast lesions verified by surgery or by biopsy. All the lesions were evaluated by plain and contrasted MRI. The lesion detectability of T1WI, T2WI, and contrasted series, the signal intensity character, enhancement pattern, morphology manifestation of the lesions were observed. Results: There were significant difference between benign and malignant tumors on enhancement homogenity, morphology and margin characteristics. Among the patients of breast cancer, typeⅠcurve was showed in 59 lesions in 29 cases, typeⅡin 12 lesions in 8 cases, and no one showed as typeⅢorⅣ. Among the patients of benign breast tumor, typeⅡcurve was showed in 6 lesions in 4 cases, and typeⅢwas showed in 17 lesions in 14 cases.ExperimentⅢApplication of MRI in follow-up after conservation therapy for breast cancerObjective: To investigate the value of MRI in follow-up after conservation therapy for breast cancer. Methods: 16 patients were enrolled in this study who experienced conservation therapy for breast cancer. All the lesions were evaluated by plain, DWI and contrasted MRI. The signal intensity characteristic, enhancement pattern, morphology manifestation of the scar and the recurrent breast cancer after conservation therapy were observed. Results: There was no significant difference between the scar and the recurrent breast cancer on the routine MRI and DWI, but there were significant difference between and malignant tumors on enhancement homogenity, morphology and margin characteristics. TypeⅠand typeⅡcurve was showed in the recurrent breast cancer, and typeⅢcurve was showed in scars.Conclusion:1. MRI can display the internal structure and relationship between breast tumor the organs around it. There are significant difference between breast cancer and benign breast tumor on DWI. 2. There were significant difference between benign and malignant tumors on enhancement homogenity, morphology, margin characteristics and type of TIC curve. Dynamic contrast enhanced MRI combined with routine MRI and DWI can differentiate breast cancer from benign breast tumor efficiently.3. There were s significant difference between scar and recurrent breast cancer on enhancement homogenity, morphology, margin characteristics and type of TIC curve. MRI can differentiate local recurrence from postoperative scar after breast conservation therapy.
